Prayer League Enrollment

PLEASE READ the following directions prior to completing the form.

1. Select donation amount. $5 per enrollee is suggested.
2. Let us know for whom to pray, listing specific intentions if you like.
3. If you'd like cards, please select "yes" in "requesting more cards?" box. If you do not need any, select "no". This step is required.
4. Check the boxes for which cards you would like. As a default, we typically send you 2 cards of whatever occasion you choose.
5. Provide your name and contact information so that we can keep a record of your requests for you.
6. Your choices of cards include:
   • Birthday
   • Healing
   • In Loving Memory
   • Special Day (Great for Anniversaries)
   • Spiritual Bouquet (General Intentions)
   • Sympathy
   • Thank You
